Spring Registration for Fall 2012

 at the McDonald's on
 Speedway and Alvernon and Grant and Silverbell

Returning Players:
Read, sign, and return a copy of AYSO's CDC Information Sheet.
Just show up to registration.

New Players:
Please Pre-register at www.eAYSO.org.
(Select Region 350).
Print 2 copies of form and bring with birth certificate or other proof of birthdate.
Players must be between 4 and 16 years old on July 31, 2012 to be eligible.
